Just under failure, right on it or beyond it?

How do you normally train?

I go to regular failure on most sets and beyond it on the last set.

depends on frequency. if you train a muscle group often, go past failure just once in a while. if you train a muscle group once per week, go past failure on every workout until you eventually feel that you need a break.
